• Employees must report the accident immediately to their employer or someone in authority.

• Employers are required to investigate the circumstances of the accident and submit a written report to the NIS within 10 working days.

• Employees should visit their doctor immediately, or go to the hospital to get the Medical Report Form certified.{{more}}

• Employees are required to complete and sign the Claim Form and submit it to the NIS Office within 15 days from the date of the accident or development of the disease.

• The receipts of all medical and traveling expenses must be attached to the claim.

• Please inform

the NIS Office if your Doctor recommends that you travel overseas for medical attention.
